《極闘5之雙殺》故事從警員Joe（李泳豪飾）與媽媽雲姐（雪梨飾）兩人相依為命開始，视频雲姐每晚經常出現發噩夢與手震的情況。Joe與女友Yoyo（吳堯堯飾）上司張Sir（梁成致飾）等一起調查黑幫坐館鄭榮的案件，视频並且找出鄭榮身邊的黑幫成員當上污點証人。可是，一眾污點成員卻一個又一個被暗殺。但更離奇的是，那些殺手又會一個又一個被殺。張Sir與Joe這一組受到上頭的壓力需要破案，然而Joe發現自己媽給媽發噩夢的情況加劇。正當Joe憑著線人「死仔輝」跟蹤外藉殺手得到線索時，卻發現另有內情。
